Desde o in\u00edcio da pandemia de Covid-19, h\u00e1 um debate significativo em torno do uso de ventiladores mec\u00e2nicos em indiv\u00edduos com quadros de insufici\u00eancia respirat\u00f3ria hipox\u00eamica aguda causada pelo novo coronav\u00edrus. Por um lado, m\u00e9dicos argumentam que, embora seja capaz de salvar vidas, essa interven\u00e7\u00e3o tamb\u00e9m poderia resultar em danos aos pulm\u00f5es j\u00e1 debilitados dos pacientes \u2014 \u00e9 o caso da chamada les\u00e3o pulmonar induzida por ventila\u00e7\u00e3o mec\u00e2nica (LPIVM). Por outro, profissionais alertam para o risco de preju\u00edzos semelhantes que surgiriam ap\u00f3s esfor\u00e7os elevados de respira\u00e7\u00e3o espont\u00e2nea n\u00e3o assistida \u2014 \u00e9 a chamada les\u00e3o pulmonar autoinfligida pelo paciente (P-SILI).<\/p>\n

Evid\u00eancias cl\u00ednicas diretas que associam o aumento do esfor\u00e7o inspirat\u00f3rio causado pelo Sars-CoV-2 \u00e0 les\u00f5es pulmonares, no entanto, ainda s\u00e3o escassas. Mas um estudo publicado no peri\u00f3dico cient\u00edfico Annals of Intensive Care no \u00faltimo dia 13 de julho traz algumas pistas sobre a hip\u00f3tese.<\/p>\n

Conduzida por pesquisadores da Universidade de Warwick<\/a>, na Inglaterra, a investiga\u00e7\u00e3o usou um modelo computacional criado para simular as caracter\u00edsticas fisiol\u00f3gicas e cardiopulmonares de 10 pacientes com insufici\u00eancia respirat\u00f3ria hipox\u00eamica aguda causada por Covid-19, em tratamento com oxig\u00eanio suplementar.<\/p>\n

O objetivo da equipe era quantificar as for\u00e7as mec\u00e2nicas que poderiam resultar em les\u00f5es pulmonares, considerando diferentes n\u00edveis de esfor\u00e7o respirat\u00f3rio, que inclu\u00edram desde um volume corrente (profundidade da respira\u00e7\u00e3o) de 7ml\/kg e frequ\u00eancia respirat\u00f3ria de 14 respira\u00e7\u00f5es por minuto \u2014 o que representa um quadro de respira\u00e7\u00e3o normal \u2014 at\u00e9 um volume de 10ml\/kg e frequ\u00eancia de 30 respira\u00e7\u00f5es por minuto, o que corresponde a um cen\u00e1rio de alto esfor\u00e7o respirat\u00f3rio.<\/p>\n
